yes you can start by reading newspaper. Go for the editorial column it is the best source to improve the English grammar.

the best way is to look for an entrance exam and then its syllabus. looking at the the syllabus will t ell you what topic to target.

yes the word based questions are not accepted. But if they are a part of the exam(covered in exam syllabus) I think they can be accepted only if you send a mock exam. Otherwise you can question on any other topic.
